James H. Tyack, DMD FAGD MAGD
Dr. Tyack is owner and President of Tyack Dental Group. A native Oregonian, Dr. Tyack graduated with honors from Oregon Health Sciences University: School of Dentistry in 1976. Immediately after graduation he served in the Indian Health Service in Arizona and in 1977 established his first practice in Clatskanie, Oregon. In 1991 Dr. Tyack was awarded Fellow of the Academy of General Dentistry status and in 1995 began serving as a manuscript reviewer for the Academy of General Dentistry publication "General Dentistry". In 1996 Dr. Tyack was awarded Master of the Academy of General Dentistry status. Dr. Tyack has served as adjunct faculty at Oregon Health Sciences University School of Dentistry.
In 2001 Dr. Tyack established his second practice location is Astoria, Oregon.
In 2010 Oregon Governor Theodore R. Kulongoski appointed Dr. Tyack to serve on the Health Services Commission and in 2012 Oregon Governor and physician John A. Kitzhaber appointed Dr. Tyack to the Health Evidence Review Commission.
In 2014 Dr. Tyack was awarded the Lifelong Learning and Service Recognition (LLSR) Award by the Academy of General Dentistry. This award recognizes Dr. Tyack's commitment to continuing education and service to his community and profession. Only 245 of the 38,000 members of the Academy of General Dentistry have received LLSR since it was introduced in 2005.
Dr. Tyack is committed to opening his practices to all in the communities he serves and providing the highest quality of care possible.
Dr. Tyack and his wife Lisa enjoy many outdoor activities and spending time with their son and extended family.
Dr. Green
"My goal is for all my patients to achieve oral health. It's my job to help them achieve this by caring, listening, and overcome any and all obstacles that stand between them and a beautiful, healthy smile".
Dr. Green has been practicing dentistry since 2010. In his spare time, he loves being with his family, which includes lots of time at the beach, hikes and searching for bugs.
Dr. Green's favorite thing to read is Calvin and Hobbes, his favorite TV show is Justified, and his favorite movies are The Burbs and any James Bond or comic book movies. For vacation, his favorite destination is Disneyland. He's also a huge fan of any sports involving the University of Utah.

Dr. Flowers
Dr. Flowers is a new associate dentist here at Tyack Dental Group. Dr. Flowers grew up in the Pacific Northwest, attended college at the University of Louisville in Kentucky, and is now returning to the Northwest as a general dentist. Dr. Flowers graduated with honors while receiving her Bachelors in Biology from the University of Louisville, She then went on to earn her DMD degree with Magna Cum Laude honors from the University of Louisville School Of Dentistry, where she received several prestigious Academic and Clinical Excellence Awards. Dr. Flowers is currently a member of Omicron Kappa Upsilon and the American Dental Association. In her spare time Dr. Flowers like to be outdoors, hiking and traveling with her husband, daughter and dog, Ziggy. She also loves all music and the guitar. Dr. Flowers enjoys giving patients a smile they can be proud of and prides herself in going above and beyond to make patient appointments enjoyable. Her goal is to continue to develop with the times, keeping her work as advanced and thorough as possible
Curtis Felman, DDS
Dr. Felman is the newest addition to Tyack Dental Group. He received his BS in business management from Brigham Young University-Idaho and his DDS from the University of Detroit Mercy. Dr. Felman's highest priority is to provide compassionate care by creating a supportive and comfortable treatment environment. He creates this environment with his personable nature and calm demeanor. He strives to provide the best oral care for his patients through his attention to detail and conservative approach.
Dr. Felman was raised in West Valley City, Utah. He enjoys spending time in the outdoors camping, rafting, and canoeing. He and his wife have two active boys. Dr. Felman enjoys taking his family to the beach and on hikes. He feels right at home in Oregon and is looking forward to helping his patients achieve healthy, beautiful smiles.
Dr. Cefalo
Dr A Jon Cefalo is an associate dentist at Tyack Dental Group. Dr Cefalo grew up in Brigham City, Utah and graduated with a bachelor of science degree in chemistry from the University of Utah. He continued his education by earning a DMD from the University of Louisville school of dentistry located in Louisville, Kentucky.
His number one goal for patients is to feel confident and comfortable with their smile. By having good oral healthcare and comprehensive treatment patients can have the smile they've been looking for. Through Dr Cefalo's patience and expertise, patients will feel at ease and confident about being able to care for their mouth.
Aside from being passionate about dentistry, Dr Cefalo loves spending time with his wife and four children. He loves the holidays, the beach, sports, and woodworking.
